    · 36.4% Were readmitted within a week if the home healthcare agency was brought in 4 days after leaving the hospital, with an average of one visit per week.

    · That rate declines dramatically to 15.5% for patients who received home care more quickly, with more frequent planned visits of two or more per week.

    · With immediate planning and three home healthcare visits per week the rate falls further to 11.4%
